Please contact your insurance agent for a Certificate of Liability Insurance. The certificate must include the following specific information:

Troy, Michigan 48083

Mid America must also be named as "Additional Insured/Loss Payee as their interest may appear."

For All Rentals -
You must provide proof of General Liability Insurance, with coverage at full replacement cost.

If You're Renting Vehicles -
You need to provide proof of Hired Auto/Non-Owned Auto coverage and Physical Damage Coverage for an amount appropriate for the truck(s) being rented.
