The Rental Agreement

Please note this is our standard rental agreement for commercial and still work. If you are a feature film, please contact our office for a copy of the Feature Film Check List and Renal Agreement.

Are you Tax Exempt?

Are you exempt from Michigan Sales Tax? Download our Sales Tax Exemption form. We need your signature on this one too, so please print the form, complete all information, and Fax it back to us at 248.528.0373.

Insurance Requirements

Please contact your insurance agent for a Certificate of Liability Insurance. The certificate must include the following specific information:

Troy, Michigan 48083

Mid America must also be named as "Additional Insured/Loss Payee as their interest may appear."

For All Rentals -
You must provide proof of General Liability Insurance, with coverage at full replacement cost.

If You're Renting Vehicles -
You need to provide proof of Hired Auto/Non-Owned Auto coverage and Physical Damage Coverage for an amount appropriate for the truck(s) being rented.
