Setting Up an Account Ready to open an account with Mid America? These forms will help you get started. Just follow the steps below and you'll be ready to roll! 	Step 1 - Rental Agreement Download and print a copy of our Rental Agreement. We need your signature on file, so after you've read the agreement, please sign it and Fax it back to us at (248) 528-0373. Download the Rental Agreement.

	Step 2 - Credit Application Download a printable Credit Application. If you choose to user the printable form, please complete all the information on the form, then sign it and Fax it to us at (248) 528-0373.

	Step 3 - Insurance Requirements We'll need a certificate from your Insurance Agent. Please read through our Insurance Requirements.

	Step 4 - Sales Tax Exemption Are you exempt from Michigan Sales Tax? Download our Sales Tax Exemption form. We need your signature on this one too, so please print the form, complete all information, and Fax it back to us at (248) 528-3800.
